1. My Health

What is my health score and what is it comprised of?

Your health is score is a combination of your Vital Score, Adherence Score and your Compliance Score


What is my Vital Score and how is it measured?

The Vital Score is calculated based off of device readings and compares those to the thresholds set by your provider. The higher the vital score, the more in sync your readings are with what the provider would deem a healthier patient. 


What is my Adherence Score and how is it measured?

The Adherence Score is calculated based on how frequently and consistently you take your prescribed medicine. If you take your medicine 10 out of 10 times, then you will have 100 for adherence. If you take it 7 out of 10 times, it will be a 70 and so on and so forth.


What is my Compliance Score and how is it measured?

The Compliance Score is calculated based off of how frequently and consistently you take your readings. A good compliance score would mean that you are taking your device measurements on a daily basis. A bad score would mean that you take your measurements inconsistently.


How would I check these scores in detail?

  1. On the bottom menu, tap on Home, then My Health Score at the top of the screen
  2. From My Health Score, you can see all three score types and can see them for periods of the previous week, previous month or previous three months.
  3. Tap on Vitals Scores to see your measurements, Adherence Score to see how many pills you have taken and missed, and Compliance Score to see a calendar view of your readings



How would I see my previous Health Records?

  1. On the main bottom menu, tap on More
  2. From here, tap on Health History (third one down)
  3. From here you can see your previous Diagnoses, Vaccinations, Allergies, Procedures, and Lab Reports


How would I add my friends or family to my Care Circle?

From the mobile app, you will not be able to add any members to your care circle. Instead you will have to login via a web browser (preferably Chrome). Once logged in, follow the instructions on User Guide for Patients (Web)


2. Devices

How do I see what devices I am connected to?

  1. On the bottom main menu, tap More
  2. From here, tap on Devices (7th one down) to see device information


Can I add my own personal fitness tracker such as as my Fitbit?

From the mobile app, you will not be able to add any personal trackers. Instead you will have to login via a web browser (preferably Chrome). Once logged in, follow the instructions on User Guide for Patients (Web)


3. Appointment Scheduling

How would I schedule an appointment?

Whether it's a a Video Consultation, an In-Person Appointment or a Personal Event, the scheduling process is nearly identical. Here's how - 

  1. On the bottom main menu, tap Schedule
  2. From here you can see upcoming appointments or history, whether they are Video Consultations, Practitioner Appointments or Personal Events. Tap on the sign at the top right of the screen
  3. From here, you will be able to add the three types of events: Video Consultation, Patient Appointment, and Personal Event
  4. Tap the appropriate type of event and fill out the form. Personal Events can be repeatable while Video Consultation and Patient Appointments will be a one time appointment pertaining to a specific subject


4. Communication

We support a variety of different communication mediums such as SecureMail, one-way Mobile Texts, Chats, and even Phone Calls. As a patient, you won't be able to send out a text, nor a phone call (the practitioner will call you), but you will be able to send out SecureMail and Chats.


If I want to send sensitive patient information, how would I do this?

For sensitive information, we built our own HIPAA compliant email system into our ecosystem called Secure Mail. To compose an email - 

  1. On the bottom menu, tap My Messages
  2. Under each user, it shows whether they are a Practitioner, Nurse, or Patient. Underneath the patient you want to secure mail, tap S-Mail and tap
  3. Enter the subject matter and the message

If you want to send out a Chat, instead of going to My Messages > S-Mail, go to Messages > Chat under the patient you want to message. Your previous chat or text history will show. Simply enter in your message and click Send. 

NOTE: Chats stay in the platform. Texts will stay in the platform and will send to the mobile device as well. 


5. Education

We have built an education library so that you may learn about your diagnoses and overall inform yourself through legitimate resources. Although, you will not be able to access the education portion on mobile app. Instead you will have to login via a web browser (preferably Chrome). Once logged in, follow the instructions on User Guide for Patients (Web)


6. Surveys

How do I access my surveys?

Surveys which have just been sent out for you to complete, will pop up on Home. Scroll down to bottom of this screen to see My Surveys. To see all of your current surveys in a list view, follow these instructions - 

  1. On the bottom main menu, tap More
  2. From here, tap on Surveys (9th one down)
  3. From here, you can tap Take Now and complete your survey(s)
