1. Patients

How do I add a new patient into the system?

As a provider, you cannot add patients through our mobile app. Instead, you will need to add patients through a web browser (preferably Chrome). To see how to add patients through web, please refer to our User Guide for Providers (Web). The mobile app will be used primarily to monitor patients on a daily basis. 


How do I update a Patient Profile? 

  1. On the left-hand side menu, click Patients
  2. From the Patients page, find the patient you wish to edit. In the same row as 


How do I add notes to a patient?

  1. On the bottom of the app, there are 5 high-level sections. Home, Patients, My Schedule, My Messages, and More. To add notes, tap on Patients
  2. From here, tap on View Details of the patient you want to add notes for
  3. Tap Notes (5th one down) and tap on the sign at the bottom right of the screen
  4. Lastly, type in your note and when ready, tap Save. Your note is now uploaded


How do I see Patient Details and what could I see?

  1. On the bottom of the app, tap on Patients
  2. From the Patients page, find the patient you wish to edit. Tap on View Details next to this patient
  3. In the Overview tab, you can see the patients Health Score. This is comprised of the Vitals Score, Adherence Score, and the Compliance Score. You can see these for the previous week, previous month or the last 3 months. To see details, tap on View Details.
  4. From Vitals & Readings, you can see all readings from the devices the customer is assigned to. These can be from the previous week, previous month or the last three months. To change the device, tap on the dropdown menu to the right of the current device (ex. Thermometer). To see the readings in a chart form, tap on the graph to the top right of the screen.
  5. From Medications, you can see the patient's medication adherence score (what percentage of their prescribed medications they are taking). This shows how many medications have been taken and how many have been missed from the previous week, month and previous 3 months
  6. From Surveys, you can see what surveys are pending for the patient, and what surveys have been completed
  7. From Notes, you can see what notes have been added to the patients and by whom (Practitioner, Client Admin, Nurses, etc). You can also add notes here.
  8. From Health Records, you could see EMR information such as past Diagnoses, Vaccinations, Allergies, Procedures, and Lab Reports
  9. From Patient Infoyou can see information such as Name, DOB, Blood Type, Height, Weight, Ethnicity, Social Security, Contact, Address and more.
  10. From More, you can see the patient's Care Plan, Consent Forms, Invoices, Thresholds, Patient Rules, Care Circle, Patient Alerts, and Programs


How do I delete a Patient Record?

For audit purposes, patient records may not be deleted. Instead, they can be Inactivated. In addition, patient records may not be inactivated from the mobile device, instead they can be inactivated from logging in via web browser (preferably Chrome). How do I update a patient's medical information? (diagnoses, allergies, vaccinations, etc)

Patient medical information cannot be added or updated through the mobile app. Please login via web and refer to the guide in the previous section to update medical information.


2. Vitals

How do I see Patient Thresholds?

Thresholds can be setup from a provider (Practitioner, Nurse) level or from the patient level. The settings at the provider level will be applied to all patients under the specific branch. The patient level will be for that specific patient. These cannot be added through our mobile app, but instead through web. Although, you will be able to see patient threshold. To see thresholds -  

  1. On the bottom of the app, tap on Patients
  2. From the Patients page, find the patient you wish to edit. Tap on View Details next to this patient
  3. Tap on Vitals & Readings
  4. Tap on the vertical ellipses on very top right. Tap on View Thresholds

NOTE: This will show you the thresholds of the device which was previously chosen. To see the thresholds of a different device, tap on the dropdown menu from the Vitals & Readings page, choose the device whose threshold you want to see, then complete step 4.


3. Medications

How do I assign Medications to patients?

Medications cannot be assigned through the mobile app. Instead they will be added by logging in via web and following the instructions on User Guide for Providers (Web)


4. Video Consultation Scheduling

How do I setup my schedule availability?

Schedule Availability cannot be assigned through the mobile app. Instead they will be added by logging in via web and following the instructions on User Guide for Providers (Web)


How do I create a new consultation or appointment?

Ex. Video Consultation

  1. On the bottom menu, click My Schedule, then +
  2. Click the Create a New Event blue button at the top right of the screen
  3. From here, you can create a Video Consultation, in person Patient Appointment, a Home Visit or a Personal Event. Tap on Add Video Consultation
  4. Enter information such as Patient, other Participants (Nurses), the Date, Time, Subject, etc.
  5. Scroll down and click Add Video Consultation. The event has now been created

NOTE: Availability must be set first in order to create a new event


How would I see my schedule?

As a provider, you can see your schedule from Home. My Schedule for Today will show as the second block. In order to see your upcoming schedule and all of your previous schedule - 

  1. On the bottom menu, click My Schedule
  2. At the top of the screen you can see Upcoming events and History


5. Communication

We support a variety of different communication mediums such as SecureMail, one-way Mobile Texts, Chats, and even Phone Calls.


If I want to send sensitive patient information, how would I do this?

For sensitive information, we built our own HIPAA compliant email system into our ecosystem called Secure Mail. To compose an email - 

  1. On the bottom menu, tap My Messages
  2. Under each user, it shows whether they are a Practitioner, Nurse, or Patient. Underneath the patient you want to secure mail, tap S-Mail and tap the "+"
  3. Enter the subject matter and the message

If you want to send out a Chat or Text, the process is nearly identical. Instead of going to My Messages > S-Mail, go to Messages > Text or Messages > Chat under the patient you want to message. Your previous chat or text history will show. Simply enter in your message and click Send. 

NOTE: Chats stay in the platform. Texts will stay in the platform and will send to the mobile device as well. 


How would I send out a phone call to a patient?

  1. On the bottom menu, tap My Messages
  2. Under each user, it shows whether they are a Practitioner, Nurse, or Patient. To the right of the patient it shows a phone icon, tap on it.
