1. My Health
What is my health score and what is it comprised of?
Your health is score is a combination of your Vital Score, Adherence Score and your Compliance Score.
What is my Vital Score and how is it measured?
The Vital Score is calculated based off of device readings and compares those to the thresholds set by your provider. The higher the vital score, the more in sync your readings are with what the provider would deem a healthier patient.
What is my Adherence Score and how is it measured?
The Adherence Score is calculated based on how frequently and consistently you take your prescribed medicine. If you take your medicine 10 out of 10 times, then you will have 100 for adherence. If you take it 7 out of 10 times, it will be a 70 and so on and so forth.
What is my Compliance Score and how is it measured?
The Compliance Score is calculated based off of how frequently and consistently you take your readings. A good compliance score would mean that you are taking your device measurements on a daily basis. A bad score would mean that you take your measurements inconsistently.
How would I check these scores in detail?
- On the left-hand side menu, click My Health
- In the Health Status section, if you scroll down you could see sections for Vitals, Adherence, and Compliance.
- The Vitals section will show all the devices you are connected to, what the thresholds are for each device and your previous readings. You will have the option to see readings by this last week, last month or last 3 months
How would I see my previous Health Records?
- On the left-hand side menu, click My Health
- To the immediate right of the Health Status section, it shows Health Records. From these high level categories, click Health Records
- From here you can see your previous Diagnoses, Vaccinations, Allergies, Procedures, and Lab Reports
How would I add my friends or family to my Care Circle?
- On the left-hand side menu, click My Health
- From the high-level categories, select Care Circle
- On the right-hand side, click on Invite in the blue box
- Choose the type, whether they are family, friend, counselor, etc. and fill out the remaining form including Full Name, Mobile Number and E-Mail. Check the consent form and click Add
2. Devices
How do I see what devices I am connected to?
- On the left-hand side menu, click My Health
- From the high-level categories, select Devices
- From here, you can see Active and Inactive Devices categorized by Tag, Name, Serial Number, Device Type and MAC Address
Can I add my own personal fitness tracker such as as my Fitbit?
Yes, we have integrations with Fitbit for personal trackers. To add the Fitbit -
- On the left-hand side menu, click My Health
- From the high-level categories, select Devices
- Click on Cloud Device on the right-hand side of the screen
- Choose your device type and click Add
- Complete the integration steps and your device will be added
3. Appointment Scheduling
How would I schedule an appointment?
Whether it's a a Video Consultation, an In-Person Appointment or a Personal Event, the scheduling process is nearly identical. Here's how -
- On the left-hand side menu, click Schedules > All
- From here you can see upcoming appointments or history from high-level categories of Video Consultations, Practitioner Appointments or Personal Events. Click on Create a New Event on the top right side of the page
- From here, you will be able to see the three types of events: Video Consultation, Patient Appointment, and Personal Event
- Check the appropriate type of event and fill out the form. Personal Events can be repeatable while Video Consultation and Patient Appointments will be a one time appointment pertaining to a specific subject
4. Communication
We support a variety of different communication mediums such as SecureMail, one-way Mobile Texts, Chats, and even Phone Calls.
If I want to send sensitive information to my provider, how would I do this?
For sensitive information, we built our own HIPAA compliant email system into our ecosystem called Secure Mail. To compose an email -
- On the left-hand side menu, click My Messages > Secure Mail
- From here, you can Compose, see you Inbox, Sent Mail, etc
- Click on Compose, choose who it's to, the subject matter and the message
- Once complete, scroll down to click on Send Message
If you want to send out a Chat or Text for short messages such as "I'm on the video consultation", the process is nearly identical. Instead of going to My Messages > Secure Mail, go to Messages > Text or Messages > Chat. Find the provider and your previous chat or text history will show. Simply enter in your message and click Send.
NOTE: Chats stay in the platform. Texts will stay in the platform and will send to the mobile device as well.
5. Education
We have built an education library so that you may learn about your diagnoses and overall inform yourself through legitimate resources. In order to access the library, click on My Library > Education. From here you could look up articles by alphabetical order, by categories or by looking them up in a search bar.
At the bottom of every article is information on who the medical reviewers are and when the last review was conducted.
How would I check for media which is sent to me by my practitioner?
- On the left-hand side menu, click My Library > Media
- Click on the high level category of Media Shared With Me. From here, you can check all media which has been shared with you
6. Surveys
How do I access my surveys?
Surveys which have just been sent out for you to complete, will pop up as soon as you login. For others, which you may have missed, you can access them like this -
- On the left-hand side menu, click My Health
- From the high-level categories, select Surveys
- From here, you can click Take Survey and complete your survey
